oneHealth is taking part in the Patient Centered Medical Home (PCMH) initiative, a new way of managing your health care. It is a model of care designed to improve the coordination of your health care with an emphasis on your all-around well-being.

We will work with other healthcare providers to take care of you. As your care team, we will involve you in decisions about your health and health care, and thus be able to develop a stronger relationship with you resulting in better care.

oneHealth encourages you to access our secure online patient portal. This patient portal will give you access to your medical records and the ability to communicate with our care team. Our care team will also be able to communicate with you via this portal any results of the care that you have received as well as educational materials that may be helpful to you.

We believe that everyone should have access to quality health care. We offer our sliding fee scale for all of our services to all income eligible, uninsured or under-insured patients based on your income and family size (Please see the link below for details of our sliding fee scale).

To provide affordable, confidential, quality reproductive health services that respect, empower and educate individuals, families, and communities. Custer County Family Planning offers most of our services on a walk in basis during regular office hours.
